Treatment Plan Instructions
Asthma and Allergy: Please complete the forms in their entirety with your child's healthcare provider. Some areas are for the parent to complete, others are for the healthcare provider. Medications are to be brought to the school with the completed treatment plan directly from a parent or guardian to the nurse. Please don't send medications or plans with the child placed in their backpack, lunchbox, etc.
PLEASE REMEMBER:
- Every plan expires on the last day of the school year. New plans and medications should be brought in on the first day of school. Schedule your appointments early for August, so your child(ren) can have a healthy start to the new school year.
- Children may be excluded from school or field trips if proper documentation and medications are not present in the school.
- If your child no longer has a condition requiring a treatment plan or medication, have your child's healthcare provider state this on an official letterhead or prescription notepad.

Please don't send medications or plans with the child placed in their backpack, lunchbox, etc. Children will be excluded from school if these documents and medications are not present, as this is a safety issue for your child. Please make your appointments early for August with their neurologist to prepare everything for the first day of classes. A seizure action plan is required even if your child does not require emergency medications or only takes medicines at home. A plan must still be in place as prescribed by their neurologist.

For your child's safety, any documented cardiac history must have an annual Cardiac Report and Limitations completed every school year. Please have your child's cardiologist or pediatrician complete this form on the first day of every school year.

You can use this form if your child requires other medication(s) during school hours, even for a short period. Please have the healthcare provider complete this form. A parent/guardian must bring this form to the nurse with the medications in the original container and the pharmacy label attached. These orders expire on the last day of the school year and must be renewed for the following year.
